Comprehensive Blood Testing

Blood work is one of the most valuable tools we use to understand what's happening inside your body, but it's only one piece of the puzzle. Rather than relying on a limited screening panel, we may recommend more comprehensive testing based on your symptoms, health history, and goals.

Depending on your individual needs, your laboratory evaluation may include a comprehensive thyroid panel, metabolic markers such as fasting insulin, glucose, hemoglobin A1c, a deeper dive than basic cholesterol, inflammatory markers, vitamin and mineral levels, iron studies, hormone testing, immune markers, and other specialized tests when appropriate.

At Fit Logic Functional Medicine, we don't treat laboratory values in isolation, and we don't make recommendations based on a single number. We interpret your results in the context of your symptoms, medical history, lifestyle, body composition, and overall health to identify patterns that may help explain why you're experiencing the way you do.

Functional Lab Testing

Saliva Testing

Saliva testing offers unique advantages for measuring certain hormones, particularly cortisol and free (bioavailable) hormone levels. Unlike blood testing, which captures total hormone levels including those bound to proteins, saliva measures the hormones actually available for your body to use.

One of the most valuable saliva tests is the Cortisol Awakening Response (CAR). This test involves collecting multiple saliva samples throughout the day - including immediately upon waking and at timed intervals - to map your cortisol rhythm. A healthy cortisol pattern shows a robust morning surge followed by gradual decline throughout the day. Disruptions to this pattern can reveal HPA axis dysfunction, chronic stress adaptation, or adrenal fatigue patterns that a single blood draw would completely miss.

Saliva testing is particularly useful for evaluating chronic fatigue, sleep disturbances, anxiety, burnout, and conditions where stress hormone patterns play a central role.

DUTCH Testing (Dried Urine Test for Comprehensive Hormones)

The DUTCH test represents one of the most comprehensive hormone assessments available today. This advanced testing method uses dried urine samples collected over a 24-hour period to evaluate not just your hormone levels, but how your body metabolizes and processes those hormones - information that blood and saliva testing simply cannot provide.

DUTCH testing measures over 35 different markers including estrogen metabolites (which reveal whether you are metabolizing estrogen through healthy or potentially problematic pathways), progesterone metabolites, testosterone and its metabolites, DHEA, cortisol patterns throughout the day, cortisol metabolites, melatonin, and organic acids related to B-vitamin status and neurotransmitter metabolism. This level of detail is particularly valuable for conditions like PCOS, endometriosis, hormone-related mood disorders, perimenopause, and when optimizing hormone replacement therapy.

Because urine concentrates hormones (containing roughly 1,000 times more estradiol than saliva, for example), DUTCH testing can detect subtle imbalances that other methods miss. It is especially useful for understanding the full picture of hormone health and guiding precise, personalized treatment.

Comprehensive Gut Testing

Research increasingly confirms what functional medicine has long recognized: gut health influences virtually every aspect of overall health - from immune function and hormone balance to brain health and energy production. Comprehensive stool testing allows us to evaluate the complex ecosystem within your digestive tract and identify imbalances contributing to your symptoms.

GI-MAP (GI Microbial Assay Plus)

The GI-MAP test uses quantitative PCR technology to provide a detailed analysis of your gut microbiome. Unlike older culture-based methods, qPCR can detect organisms that are difficult to grow in a laboratory setting, providing a more accurate picture of what is actually living in your gut. This test identifies pathogenic bacteria, parasites, H. pylori (including virulence factors), yeast and fungal overgrowth, and viral markers. It also measures beneficial bacteria levels, inflammatory markers like calprotectin, digestive function markers including pancreatic elastase, and immune markers like secretory IgA.

GI-MAP results help us create targeted protocols for conditions like IBS, SIBO, chronic bloating, food sensitivities, autoimmune conditions, skin issues, mood disorders, and chronic fatigue - many of which have roots in gut dysfunction that standard testing never reveals.

Cyrex Arrays (Immune and Food Reactivity Testing)

Cyrex Laboratories specializes in functional immunology and autoimmunity testing, offering panels that go far beyond standard food sensitivity testing. Their Array 2 (Intestinal Antigenic Permeability Screen) evaluates leaky gut by measuring antibodies against zonulin, occludin, and other tight junction proteins - providing objective evidence of intestinal barrier breakdown. Array 3X provides the most comprehensive gluten sensitivity assessment available, testing for reactions to multiple wheat proteins and identifying gluten-related autoimmunity that standard celiac panels miss.

Array 4 identifies foods that cross-react with gluten - meaning foods that can trigger a gluten-like immune response even though they do not contain gluten. This is essential for patients who remain symptomatic despite a gluten-free diet. Array 10 measures immune reactivity to 180 different foods in raw, cooked, and modified forms, helping identify dietary triggers of inflammation and autoimmune reactivity. For patients with complex autoimmune conditions, Array 5 screens for predictive antibodies against multiple tissue types, sometimes detecting autoimmunity years before clinical disease develops.
Cyrex testing is particularly valuable for autoimmune conditions, unexplained inflammation, complex food sensitivities, and patients who have not responded to standard elimination diets.

Safety

Lab testing is generally safe, but a few considerations apply. If you have a bleeding disorder or take blood thinners, let us know before any blood draws. Some tests require specific preparation - fasting, collecting samples at certain times, or temporarily discontinuing supplements - and we will provide clear instructions for each test you need. While specialty tests like DUTCH and GI-MAP provide valuable functional insights, they are not intended to replace diagnostic testing for acute medical conditions. If you experience severe or worsening symptoms, seek appropriate medical evaluation. Lab results are one piece of the puzzle; we always interpret them in the context of your complete clinical picture.

FAQs

Why can I not just use my regular doctor's lab work?

Standard lab panels provide valuable baseline information, and we are happy to review any recent results you have. However, conventional panels are designed to screen for disease, not optimize function. They often use broad reference ranges that miss early dysfunction, and they do not include many of the specialized markers - like hormone metabolites, gut microbiome analysis, or detailed immune reactivity - that functional medicine relies on to identify root causes.

How We Recommend Specialty Testing

Not everyone needs specialty testing. During your consultation, we'll determine whether additional testing is likely to provide meaningful information that will change your treatment plan. Before ordering any test, we'll explain why we're recommending it, what it measures, what it costs, and how the results may help guide your care. Because Fit Logic Functional Medicine is a cash based practice, all testing costs are discussed with you in advance so there are no surprises.

How long does it take to get results?

Standard blood work typically returns within 3-5 business days; although we run more than "standard" bloodwork and some results take up to about 10 days. Specialty tests like DUTCH and GI-MAP usually take 2-3 weeks from sample receipt at the lab. Cyrex and Genetic testing may take 3-4 weeks. We will contact you to schedule a results review as soon as your findings are in. G

Do I need to fast or prepare for these tests?

Preparation varies by test. Most comprehensive blood panels require a 10-12 hour fast. DUTCH testing involves collecting dried urine samples at specific times over 24 hours. GI-MAP requires a stool sample with no special diet restrictions but does have timing requirements for shipping. We provide detailed instructions for every test ordered.

Should I stop supplements before testing?

It depends on the test. For nutrient testing, we may ask you to stop certain supplements temporarily so results reflect your true baseline. For hormone testing, biotin supplements should be discontinued at least 48 hours prior as they can interfere with results. Gut testing generally does not require supplement changes unless you are taking antimicrobials or certain probiotics. We will give you specific guidance based on your testing plan. We recommend stopping any supplements with biotin for 3-5 days if we are testing your thyroid.

How often should I retest?

Retesting frequency depends on what we are monitoring. For hormone optimization, we typically retest 8-12 weeks after starting or adjusting treatment. Gut healing protocols may warrant retesting at 3-6 months. Some markers like thyroid and metabolic panels benefit from quarterly monitoring initially, then annually once stable. We will build retesting into your care plan based on your specific needs.

Can these tests diagnose disease?

Functional medicine testing is designed to identify dysfunction and guide treatment optimization - not to diagnose medical conditions in the conventional sense. If our findings suggest a condition requiring formal diagnosis or specialist evaluation, we will coordinate appropriate referrals. Our testing complements, rather than replaces, conventional medical evaluation when indicated.

Do you offer telehealth appointments for testing review?

Yes. While some testing requires in-person sample collection, many specialty tests can be completed at home with a kit we ship to you. Results review appointments are available via telehealth for patients throughout Massachusetts, making it convenient to discuss your findings and treatment plan from home.
